Eldercare planning is complex, and in most cases, it involves more than just filing a Medicaid application. Rather, there is often a need for a comprehensive care plan to be in place before the application to establish a roadmap to sustainable security. Having a trusted guide to help you understand your options is the key to feeling confident that you are making the right choice for yourself or your loved one’s care.

In partnership with Callahan Care Solutions, a personalized life care planning service, we are proud to offer the Life Care Plan as a preliminary step toward long-term care. This comprehensive service is designed to guide families through the evolving landscape of long-term care with clarity, compassion, and actionable support. This plan is particularly valuable for individuals with chronic illnesses or cognitive decline who are navigating complex medical, financial, and social challenges.



What Can You Expect

1. A Comprehensive Evaluation

Each Life Care Plan begins with a detailed intake process, which includes direct engagement with the client and responsible party, a full review of medical history, and environmental and psychosocial assessments. This approach ensures we capture the full picture — physical, emotional, financial, and logistical.

2. Care Coordination & Medical Advocacy

The Callahan Care Solutions team will identify the client’s full care team and collaborate across providers to streamline communication and reduce care fragmentation. This includes reviewing diagnoses, medications, durable medical equipment, and in-home safety measures.

3. Finance & Benefit Mapping

Callahan Care Solutions will assess available income, assets, and current benefits (e.g., Medicare, VA, community resources) to create a realistic roadmap for care. This framework evaluates eligibility for additional support programs and provides guidance on benefit navigation and planning for future care transitions.

4. Cost Projections & Care Planning

The plan outlines anticipated care costs over a 1–5 year horizon (situation dependent), including direct, indirect, and long-term care expenses. Options such as in-home support, respite services, and memory care transitions will be detailed with timelines based on clinical trajectory and caregiver capacity.

5. Strategic Recommendations

Clients and families receive tailored, actionable recommendations, such as applying for specific benefits, increasing respite, or beginning Medicaid spend-down strategies. Guidance on working with elder law attorneys is also available when needed.



What is Included

Initial Consultation & Needs Assessment

● In-depth review of the client’s health status, financial situation, family dynamics, and goals.

Custom Life Care Plan

● A written roadmap that outlines recommended care settings (in-home, assisted living, memory care, skilled nursing, etc.), financial options, and support strategies based on the client’s unique circumstances.

Resource Navigation & Planning Guidance

● Education on public and private pay options (Medicare, Medicaid, VA benefits, long-term care costs), and referrals to appropriate community providers.

Ongoing Case Management Support

● Four months of direct access to a dedicated case manager for continued decision support, care coordination, and crisis navigation.



What is the Process

During a free consultation with a member of our team, we will help to weigh out your options regarding long-term care planning. If the Life Care Plan aligns with the goals you have for yourself or your loved one, and you engage in our service, we will coordinate the initial planning call with Callahan Care Solutions within 1-2 business days.

During the first planning call, your goals, questions, and concerns will be reviewed by an expert care manager. This call is typically a 60-90 minute in-depth conversation conducted by video conference or phone call to review:

● Medical and functional needs

● Living arrangements and safety concerns

● Financial considerations and benefit eligibility

● Family dynamics, caregiving roles, and long-term goals

The next stage of the process is drafting the Life Care Plan. Once the planning call has been completed and documentation has been received, the Life Care Plan is created. A follow-up conference call is conducted to review the plan with the individual or family, confirm details, address questions, and verify any corrections that need to be made. Within approximately six weeks of the first planning call, the finalized Life Care Plan is delivered.

Throughout the process, you will have continued support and access to the case manager assisting you. There is no limit to the number of times you can message the case manager, and there will be four monthly scheduled check-in calls after the delivery of the Life Care Plan. During these check-in calls, topics such as the following will be discussed:

● Navigating provider options and referrals

● Family education and decision-making support

● Crisis planning and care transition guidance

During the final check-in call, a summary of services will be provided, and an opportunity to discuss annual review options and ongoing care consultations will be provided. Ultimately, you will walk away with a roadmap to the future and a comprehensive understanding of what actions to take as milestones are approached.
